Friend of Adapted Physical Education Nomination Form

Background: The Friend of Adapted Physical Education Award recognizes individuals who have demonstrated sustained advocacy for Adapted Physical Education by continually promoting quality adapted physical education and equal access across the continuum of services for students with disabilities in North Carolina. This award recognizes a general physical education teacher, exceptional children teacher, teacher assistant, administrator, university professor, or general community member. The recipient has demonstrated sustained support of students with disabilities learning the skills necessary to remain physically active and pursuing meaningful lifetime leisure and recreation in their communities.

Criteria:

  • Membership in NC SHAPE is not required for the Friend of APE Award recipients.
  • Currently demonstrates/has demonstrated advocacy in the field of APE (school or community based)
    • Displays/has displayed leadership in promoting high-quality, inclusive PE and fostering lifelong participation for all participants whether at the school, district, or community level. This is most concerned with visibility, influence, and driving change.
  • Currently promotes/has promoted the implementation of inclusive practices and equitable access
    • Actively implements/has implemented strategies to removes barriers and ensure students with disabilities have meaningful opportunities in physical education and/or supportive community based programs. This is more about hands-on implementation and direct impact on students.

Nomination Deadline: April 15 of each year.

Nominee Requirements: 

  • Complete an Application and Upload the Application Documents: Once the Recognition Committee chair receives the nomination, an email will be sent to the nominee with a link to the application form, where the 1-page vita and written statement can be uploaded. All documents must be in PDF format.

  • Curriculum Vita: Prepare a 1-page curriculum vita highlighting their experiences relative to the award criteria above.

  • Written Statement: Compose a 250-word statement summarizing your qualifications for this award.

Application Deadline: May 15 of each year.
