Linda Harrill Rudisill Lifetime Achievement Award Nomination Form

Background: The Linda Harrill Rudisill Lifetime Achievement Award recipients are master teachers, advocates, and innovators who have demonstrated lifelong achievement and dedication to health education.

Membership in the NCAAHE and NC SHAPE is not required of NCAAE Lifetime Achievement Award honorees.

Award Criteria: Recipients of the Linda Harrill Rudisill Lifetime Achievement Award shall be:

  • Master teachers with 25+ years of teaching health education.
  • Advocates and innovators who have demonstrated outstanding achievement and noteworthy dedication to health education.
  • Persons of high moral character and personal integrity.

Nomination Deadline: April 15 of each year.

Nominee Requirements: 

  • Complete an Application and Upload the Application Documents: Once the Recognition Committee chair receives the nomination, an email will be sent to the nominee with a link to the application form, where the 1-page vita and written statement can be uploaded. All documents must be in PDF format.

  • Curriculum Vita: Prepare a 1-page curriculum vita highlighting their experiences relative to the award criteria above.

  • Written Statement: Compose a 250-word statement summarizing your qualifications for this award.

Application Deadline: May 15 of each year.
