Physical Education Hall of Fame Nomination Form

Background: The North Carolina Physical Education Association recognizes top physical educators from North Carolina who have pioneered the physical education field as we know it today. Inductees will be enshrined in the North Carolina Physical Education Hall of Fame and will receive a personal Hall of Fame plaque. Their names will be engraved on the perpetual Hall of Fame plaque displayed yearly at the NC SHAPE Annual Convention.

Hall of Fame Guidelines:

  • The nominee must have taught in North Carolina for at least five years.
  • The nominee must be retired from the teaching profession for at least 5 years.
  • The nominee must have conducted a successful and unique physical education program, promoted a healthy lifestyle for students inside/outside the classroom, and promoted/advocated the physical education profession.
  • This award reflects the candidate’s entire scope of work as an educator and service to the profession.
  • The Hall of Fame Nomination Form must be completed by a peer or themselves.
  • A PEA board member can also nominate a candidate to the board for approval.
  • Nominations will carry over from year to year if not selected.
  • After the inaugural class of 2021-2022, there will be 2 members inducted each year.
  • Members will receive a Hall of Fame plaque at the annual state convention.
  • Each inductee’s name will be engraved on the perpetual Hall of Fame plaque displayed yearly at the NC SHAPE Annual Convention.

Award Criteria:

  1. Career Longevity & Commitment: Years of dedicated service to physical education and sustained excellence over time.

  2. Impact on Students: Demonstrated lifelong influence on student’s physical literacy, character development, leadership, and healthy lifestyles.

  3. Leadership & Mentorship: Leadership within school, district, state, or national organizations.

  4. Lasting Legacy: Evidence that their work continues to influence students, staff, or programs beyond their active service.

  5. Character & Professional integrity: Modeling sportsmanship, ethical conduct, and serving as a positive ambassador for physical education.

Post-Nomination Procedure: Once the Recognition Committee chair receives the nomination, an email will be sent to the nominee with a link to the application form where the 1-page vita and written statement can be uploaded.

Nomination Deadline: April 15 of each year.

Required Application Documentation: The following documents must be uploaded in PDF format.

  • The completed Hall of Fame Nomination Form.
  • A recommendation letter from a colleague.
  • A written statement on how the nominee conducted a successful and unique physical education program, promoted a healthy lifestyle for students inside/outside the classroom, and promoted/advocated the physical education profession.
  • A current curriculum vitae or resume.

Application Deadline: May 15 of each year.
