NC SHAPE Advocacy Award Nomination Form
Background: Recognizes persons or organizations for affecting educational policy and legislation through effective advocacy at the local, state and/or national levels. Recipients do not have to be members of NC SHAPE.
Qualifications for the Advocacy Award:
- Candidates shall have an outstanding record of advocacy affecting educational policy and legislation in one or more of NC SHAPE’s allied fields.
- Candidates cannot be current members of the NC SHAPE board of directors.
Nomination Deadline: April 15 of each Year
Nominee Responsibilities: The nominee is responsible for preparing a 1-page curriculum vitae and a 250-word written statement highlighting their involvement in advocacy in one of NC SHAPE’s allied fields.
- Curriculum Vita: Prepare a 1-page curriculum vita highlighting your involvement in advocacy in one of NC SHAPE’s allied fields.
- Written Statement: Compose a 250-word statement summarizing your involvement in advocacy in one or more of NC SHAPE’s allied fields.
- Complete an Application and Upload the Application Documents: Once the Recognition Committee chair receives the nomination, an email will be sent to the nominee with a link to the application form, where the 1-page vita and written statement can be uploaded.
Application Deadline: May 15 of each year.
